Vineyard Resource Center

- Services: Showers, laundry, food 12pm-5pm, on-site services scheduled on various days (housing, health, free phone). On-site housing navigation through Housing Consortium of East Bay: Monday, Tuesday, Thursday & Friday
Open Heart Kitchen Refuge
- Open 6PM-8AM M-Sun 450 N. Livermore Ave., Livermore. Phone number available only to those staying in the refuge.
- Overnight shelter with limited capacity (30 persons). 18 + only. Dinner and Breakfast are provided.
Cornerstone Fellowship
- Open Fridays 9AM-11AM. (925) 447-3465 Services located at the back of the Student Center. Bus stops in front of the church. Showers, laundry, hot breakfast, clothing. Safe Parking Program for women & children through referral from CityServe of the Tri-Valley.
- Connect onsite to many services including housing crisis intake, homeless prevention and intervention, primary & preventative healthcare, including registered nurse and/or nurse practitioner. Dental bus onsite 9am-4pm
Abode Services Housing Resource Center

- Intake for the county housing match system, Coordinated Entry. Housing problem solving and housing search support is provided
CityServe of the Tri-Valley

- Provides care coordination, vital document assistance, job & shelter/housing navigation, basic needs, mental health support.
Bay Area Community Valley Wellness Center (BACS)

- Breakfast, lunch, clothing, housing problem solving, employment services, linkage to community resources, nutrition and healthy eating support, social outings in the community, peer led groups that focus on skill building, coping skills, and symptom management.

Alameda County Behavioral Health Services
- Acute Crisis Care and Evaluation for Systemwide Services (ACCESS) 1 (800) 491-9099
- Telephone line staffed M-F 8:30-5 pm by licensed mental health clinicians and administrative support for both general behavioral health questions and determining eligibility for a range of outpatient services.
- Geriatric Assessment and Response Team (GART) (510) 891-5600
- Mental health assessment for ages 55 and over. Must be an Alameda County resident and have Medi-Cal or Medicare, or be uninsured. Must have a mental health need. Up to 60 days of assessment and connection to needed care providers.
- Community Assessment & Transport Team (CATT) 911 (weekdays) & 988 (Nights and weekends).
- Crisis intervention by clinicians and EMTs, 5150/5585 assessment, diversion to voluntary services, and referrals to a wide range of mental health and substance use services- County Wide. Operates 24/7, 365 days per year.
- Outreach and Engagement Teams (510 891-5600, press 2 for outreach.
- Post-crisis services available to Medi-cal and Medicare eligible, or uninsured patients in need or non-urgent follow-up care. Brokerage and linkage services to assist with coordination and navigation resources.
Alameda County Social Services Agency

- Responds to reports of abuse of Alameda County residents age 60 or older and dependent adults between 18-59 who, for mental or physical reasons, cannot protect their rights or carry out typical daily activities. Service is provided regardless of income, and acceptance of service is voluntary. Reports taken 24/7 by phone or online.

- A Christian transitional housing program that helps men struggling with homelessness due to job loss, recent release from incarceration, drugs & alcohol problems, and lack of family support.
Shepherd’s Gate

- A Christian transitional comprehensive housing services program that helps single women or women with children (girls up to age 18 years of age, boys up to 12 years of age).
Sojourner House

- A homeless transition shelter for homeless families in a safe and supportive environment, a 16-bed facility. Clients can stay up to 6 months. Serves 2-parent families, families with teenage boys, single mothers with children, single fathers with children, and single women.
Tri-Valley Haven

- A shelter for women and children who are victims of violence (domestic violence, sexual assault, human trafficking). 30 beds. Families may include boys and girls up to the age of 18.
